PERMISSION FORM FOR ONLINE POSTING OF GRADES

In order to conform to Federal privacy laws, teachers are not allowed to post grades without permission or to post them using any part of your Social Security number. This form gives me permission to post grades online at the class website. I will only use the last 5 digits of your Tech ID number to identify you, and I will post these grades online only with your permission.

• The website will be updated weekly to indicate your performance on Quizzes and to acknowledge Extra Credit points.
• You do not have to sign or return this form. If you do not, you will have to see me individually to find out your grades.
• Whether you return this form or not, you may see your individual answer sheets for any Quiz on request.

Remember, if you do not return the form or sign it, you will still have to check individually.

I give Professor Larsson permission to post Quiz and Extra Credit scores online, using only the last 5 digits of my Tech ID number to identify me.

I understand that if I do not return or sign this form, I will have to contact Professor Larsson individual to find out my grades.
